Email Check-In


1. Fill out all required forms indicated in the Team Check-In Requirements below.

2. Send a SINGLE email with ALL required documents to: Include the following in the SUBJECT line of the email:

· Team Name (Example: NRU 06 Red)

· Team Age (Example: U14)

· Team Gender (Example: Male)

3. NOTE: The deadline to submit your team information via email for Team Check-In is 12:00 PM, Wednesday, May 16th, 2018.

4. We will send an email response that you are properly checked in within two (2) business days after all documents are received.

5. Please print and bring to the fields four copies of your Team Roster.


Team Check-In Requirements

REQUIRED for team check-in:

· Roster: Please fill out our specific tournament roster form

· PDF file containing player cards/passes (fronts of cards only) for Each Player! (If you do not have player cards/passes, you are required to have copies of all rostered players' birth certificates. Please let us know this when emailing your documents.)

· Team Info Sheet

· Permission to travel form if necessary


NOTE: All Teams MUST have available on the sidelines during ALL of their games the following:

1) Medical release forms for each rostered player (your club's generic form is acceptable

2) Proof of age: official player card/passes or birth certificates

3) Team Information Sheet
